Formalism in theatre, I believe, was a term coined in the 70's.
Formalism is about structure. It is about Form. But it is about how form moves.
(I like to think that Form follows Structure.)
In music, many cite Philip Glass as a Formalist. I tend to cite Laurie Anderson. The fact that the two of them released an album together makes me happy.
Film? Imagery (Fellini) vs. the purpose of the movement of Images. To that end, Scorcese comes to mind. Specifically the opening sequence of "Goodfellas".
Literature? Some could cite Dylan Thomas but I prefer ee cummings.

Hope that helps.
